You will have to go to the BMV first to make the transfer. Then you will have to go to the title bureau and have it put in your name, a new title that is. If you go to the one at Westpointe Plaza they are next door to each other and you should spend a total of 30 minutes most at both places combined

Just your old license. If you are going to be required this go round to get an enhanced license then you should bring 3 separate pieces of mail received at your address within the last 12 months or, if your address has not changed from what's on your current license then just bring two (they can't be from the same source). You will also need a certified copy of your birth certificate as well as your social security card and proof of name change if applicable. If you have a passport you will not need any other documents except your current license. Real IDs aka EDLs will be required as October 1, 2020 in order to travel through airports within the United States. Standard DLs will no longer be acceptable forms of identification for TSA unless you also have a current passport.

No, you do not need an appointment, and honestly it's kinda a crapshoot about the best time to go. I went at like 2:00pm on a Tuesday and got in and out in less than 30 minutes.

Before you do plan on leaving the day of your choice, you go on their website and get in line online, as they call it. They'll give you a ticket number and you just go put your phone number in the kiosk (they'll direct you to it if you're not sure) once you've done that you wait until they text your phone number so you know when they're ready for you. They don't have a public fax number unless it's related to registration or reinstatement. For proof of insurance there is a fax but they only accept faxes from insurance companies. They will sometimes fax things for you to specific departments but will charge a fee of approximately $11.00. Without knowing the reason for the transmission it's difficult to direct you. There is not just one fax number. There are different fax numbers for different departments and situations.
You can go to this location. You will need to go to the title bureau next door to the BMV to officially record the title transfer (there is a fee and it's best to bring cash). When you are done at the title bureau, they will direct you next door to the BMV to get plates and registration recorded to the vehicle and your name.
