You have to actually fill out a social security card application and drop off original documents. I.e. birth certificate, photo id, proof of residency and legal documents showing your name change. I had to do it for my son yesterday. You can drop it to an office and hand it to security

Trouble? Most likely not, if it was that long ago. But if that is the extent of your work record it won't support a title II social security disability insurance claim. But you may be eligible under title XVI supplemental security. If you worked and paid into the system in the intervening years appropriately you may be eligible under title II. It depends.
