You need your ssc and birth certificate you also need 2 forms of mail stating your name and address on their as long as you have those your good to go they end up giving you a print version of your identification card then 5 to 7 business days a hard copy will come in the mail for you :) hope this helped

Birth certificate, un-laminated S.S. card (or they won't accept it) or a W-2 (from 2017, that you should have just received, tax forms aren't accepted) Utility bill in your name & Rental Lease ( to prove home address) Marriage license (if you're using a married name). And if you call the license bureau they have a number you can pre-register to save time in line but I don't remember the number they may be able to give it to you if you call before you go. I did it and it saved me like 30 minutes because I got there signed in under the registry number that they gave me on the text and I was up at the window in like 5 minutes.
