For the DL, need more than the birth certificate/Passport. Need a second form of ID (work badge w/picture, student ID, medical insurance card, etc) plus one proof of address (bill, bank statement, vehicle registration, etc)
You can take your state-issued birth certificate (NOT hospital copy with footprints), or valid passport to verify identity, to replace driver license. For your social security card, you'll have to contact the Social Security Administration to check what documents they require.
