I walked in without an appointment just before 9 am. Security guard asked what the purpose of my visit was. And he wanted a detailed explanation…I almost replied with “are you serious because it’s not your job to ask me for personal information.” The total time spent was less than 1 hour because the first employee I spoke with was able to answer my questions. And don’t forget to wear your mask properly because the mask police will remind you. Hopefully that was my first and last time I will have to go there.

IN THE LETTER, THE DOCTOR MUST SAY THAT YOU ARE UNABLE TO WORK!!! And, get a lawyer ASAP. I would reccomend Binder & Binder. They are a National Law Firm who ONLY works with Disability benefits. And see LOTS AND LOTS of specialists for your condition(s). You are in my prayers.

If name change by marriage. Take your marriage certificate to Social Security office with all legal documents that you may have. Do the same with the DMV and again the same with the DOH. I would call ahead all three offices and ask them for which specific paperwork they all require. I used my drivers license, but had my old SS card also( i needed a new one issued), so more than likely you will need your BC.. u have to get that from the health dept and they WILL charge u there, but for your SS card there is no charge. For Medicaid (Which is mainly based on income alone) go to myflorida.com and follow the site to fill out forms for Financial Assistance. Medicare however is based on either age or disability. After you win your case for disability or turn 65 you are eligible for Medicare. After one of those things happens, go to Medicare.com and read what to do next. First off, you have my condolences for your loss. Second, so that you are fully aware, the Death Benefit is only $255. And, he had to have been working full-time for a minimum of 5 years (And filing taxes). And, cannot be collecting ANY Government Benefits while in the last 12 months of his life. And, he cannot be legally married at the time or having any surviving children. AND, you would have to be named Executor of the Estate. I hope that this helps.

Credit Karma is a free way to do it. Unless you're planning on making any big purchases, I would put a 'hold' on your credit info, for your protection. Then contact all 3 Credit Bureaus and request a copy of your report. They HAVE TO give you a free report once a year at your request. I hope that there isn't anything wrong with your credit.
