Do I need to bring my birth certificate with me to apply for a new ss card? Mine was stolen and the last time i was issued one i was 15. Also do i have to pay if so how much does it cost?

I used my drivers license, but had my old SS card also( i needed a new one issued), so more than likely you will need your BC.. u have to get that from the health dept and they WILL charge u there, but for your SS card there is no charge. Go on the soc sec website before u go to health dept, they will tell u what u need to bring to get a new card.

Yes, you have to show 2 forms of identification and it wouldn't hurt to have a piece of mail postmarked within the last 30 days
