If name change by marriage. Take your marriage certificate to Social Security office with all legal documents that you may have. Do the same with the DMV and again the same with the DOH. I would call ahead all three offices and ask them for which specific paperwork they all require. Good luck, it's easier than you think as long as you have the legal documentation to prove who you are.
If you haven't yet legally changed your name, you will need to go to the courthouse and petition for a name change. If your name changed due to getting married, they USUALLY start the process of changing it when you give them a completed marriage certificate/license. If your name changed due to some other reason, you will need to go to the local courthouse and begin the process of changing your name. Just make sure that you get between 5-7 copies of the name change paperwork, because you will need to send them into the Social Security Administration, the DMV, your health and car insurance provider, as well as any other types of licenses that you have.
