Before you go to any NYS DMV, visit a local law enforcement precinct to file a lost property complaint. Ask whomever is taking your report that you need the case or complaint report number. That case or complaint# will be needed by DMV which should be written on the (small) MV-78B Form or slip. That same form can be used for lost and stolen license plates as well.
