It's a quarter mile down, quarter mile up. You go out the same way you came in. Number of stairs total inside the cave are about 70. There is a part where you go down 46 stairs to get to the last two rooms of the cavern. Your tour guide lets the group know when you're going to that part of the tour and that part is optional. You walk and stop in each room to learn a bit, then when you come back up those 46 stairs the tour takes a breather!
The walk from the Visitor's Center to the cavern has no steps. But inside the cavern are a lot of steps. The trail is VERY well developed and paved all the way. If you have trouble with the steps, you can elect to not participate in the step portion of the tour.
There is short walking trail (about 2 miles).
