You need to bring your out of state driver license, a birth certificate or passport, your ss card, and a piece of mail with your current address.

I agree with Denise. A utility bill is preferred if memory serves me correctly.

Your old drivers licenses plus a piece of mail that shows your current address on it. You can also call the dmv they'll tell you what you need.
