Uhh ur last address/ lease/ past residancy letter. A letter stating from landlord that you cannot live there anymore. Birth certificate. Social sercurity. And be aware that ur gonna have to plead ur case with them they will push ur buttons and try to turn you back but its all about patience and being strong.

A letter from the person you cant live with . They may make a visit if you are in the 5 boroughs to see the space that person had when you were staying there to see why you cant stay there with finacial assistance from NYC to see if you can return there. Along with social security cards, birth certificates, lease and any eviction notices, medicals for children and any court orders from any family court ordering you to enter there. You will be interviewed Nd stay in a temporary place for 10 days. If they find you ineligible, all family members have to return to PATH and reapply. If this happens, bring all belongings with you. Everything as they may clean out your room. If you are single, same thing. Best of luck to you and yours.....
