All you need is state issued identification. In filing out the application, provide a local address that you have access to. I still have my original id from Ohio and they associate your library card with the local address and your birthday it's really easy to obtain a card.
You just fill out a short application. It doesn't ask for crazy information either. And that's it turn it in and you can get your card that day.
