When I got my license I had a passport, social security card and a proof of my address where I lived. It did not cost anymore ..of course I was renewing my license .
If you want the gold star on your license, you need to have a birth certificate or a valid (current) passport, or other document of proof of U.S. citizenship, and 10 dollars. If you don't want the gold star, then just the 10 dollars. The gold star must be on your license if you are to board any commercial aircraft or accessing federal facilities requiring ID.
I believe it is $10 and I think you have to have 2 forms of ID.
