Maybe try calling the humane society and see if they can assist you with filling out the application over the phone. Or go in and do it in person? Sorry that's not much of an answer, but hopefully it helps.

I agree with previous comment. By going in person you can interact with available kittens and see which one is most suitable. With so many to choose from, you can see which one is ideal for you.
