Go with your birth info and pay the fee
If you have a valid state ID, that's all the identification you'll need. The applications are provided to you and theres a $23 fee. If you dont have an ID, you'll need to provide at least 2 other forms of identification, like a passport and social security card, but there are other supplementary documents you can use as well
