Does the doctor usually tell people things are free, then send them a bill? Sure are lots of reviews saying people are getting charged for things they didn't ask for.

Previous answer not very empathetic. The problem is the doctor might have to deal with lots of different insurance companies and one outlier might decide that because of how this or that was coded (even if just a PCP visit), they'll say you have to meet the deductible before they'll pay anything. Doctors don't know all that for every company, just what they generally do. Our system encourages billing everything high and then the insurance company or Medicare bargains it down. No wonder the U.S. has the highest health costs but the Canadians live 3 years longer.
