You need to document everything and take photos...then call animal control..if for some reason they wont handle it then call the sheriff/police....then if for some reason you cant get any assistance...which by law they're suppose to respond but fail to find or agree with your assessment....you need to contact the Humane Society of the United States.
Yes call animal control. They wii take it from there and try to keep you out of it.

I've called and watched Aspca show up to the neighbors house where the owner pretended to love his dog and the agent did nothing. I have watched this poor dog sit in the rain because he has no shelter, cry for days on end because his chain is tangled and because he has no food or water.. I will be calling the humane society. Glad i found this post.
