They also evaluate you every yr. To see if there's any changes in your income. You have to take light bill stubs, water bill & a copy of your income.They are very nice there though.
Yes they do, as has been said it is based on your income, i pay a copay of 20 bucks each visit, also have to pay some on any labs they may do from bloodwork and such.
