Jacket is not required, jeans are ok. Lately just about anything goes as long as it's not too torn or dirty looking. Dressing to go out to dinner is not what it was.
Depends on the evening. If it's after 7 on a Friday or Saturday I would opt for dark jeans and a collared shirt at a minimum
Definitely not mandatory, it's all up to you on what you want to wear. I had on a bottom up and khaki pants.
