How do I get information about my grandmother's death certificate? She passed away here in 1965 or 1966?

I wasn't asking about obtaining the actual certificate of death. I'm inquiring on what department of the hospital should I contact about the correct information on a certificate of death. I apologize, I should've been more detailed initially..

You don't get it at Jacobi Hospital, you get it from the Hall of Records at 125 Worth Street in Manhattan.
