I had applied for POC card back on September 2015. Every month, I keep checking the status online but cannot get any results. I have a copy of my manual receipt that I obtained from the Embassy in Washington DC where I applied. Please advise.

The NADRA office in Islamabad says that they never received the case.

There is no longer NADRA office in embassy anymore. You should call Nadra office Islamabad for advise. They will be able to assist you. Google their number
