Hi Robert, yes....if you do not renew your certification with the recommending provider, the card itself will expire. After you see the doctor to renew the recommendation you will enter the new certification number into your DOH account to renew. If you wait and the card expires before you do this, you will get a new card issued to you and the old one will be voided out for good. Hope this helps!
You need to renew your card every year. Call your Dr for an appointment to file for another year. Then go to the nys site to confirm
A small detail, you have a NY ID & Doctor's prescription. The prescription needs to be renewed yearly. The Date is on the prescription.
