If it's for newborns and children you will need their birth certificate and or shot records. Originals only. No copies. For adults you will need a state ID. If you do not have that, you will need in its place a passport , or work ID (government), or a medicare card. If you do lot have those, you will need to have a medical record that's no older than 2 years old. You can go online at www.ssa.gov for more information
