How can i get a prove or a receipt that shows that i have send a filled forms to the court?

Do i have to add my passport size photos to the form filled? And is't compulsary for me to add declaration to the forms. Please i need help, am not working because am not having permit to work so i can't hire a lawyer. Thank you very much

You mail a copy of the filled form with the original to the court. Include an envelope with your address and stamps. The court will date stamp your copy and mail it back to you. Make sure you mark your copy as respondent's copy and provide a return envelope and stamps.
