They are usually busy on weekends so I'd say yes you need to reserve on weekends unless it's just a double date or a couple of friends
Usually not. However during busiest times (Monday night, Friday night, and Saturday night) its not uncommon for every lane to be in use and its a good idea to call ahead at those times.
No. But if you're really worried, you can call in advance. Sometimes they host events there and may not have an open lane.

No, not always. It just depends how busy they are. A lot of times you can just go in and get ome. I would just call ahead and get one though. Its not really that predictable.
