If you're staying onsite in a Disney hotel (or in some of the Disney Springs Good Neighbor hotels, you can (and should) book fastpasses at 60 days. For Flight of Passage, try to book fastpasses for the end of your trip first.

You don't need to but better to, really long queue. Get fast pass as soon as and as early as possible.

Try app at least 60 days in advance is your best bet.

You can book on the app. Some people who bought tickets in advanced were able to get fast passes via the app up to 60 days or more.
