I saw Dr. O a few months ago & didn't need them but in my case, my qualifying condition was obvious. You might need them if your qualifying condition is chronic pain. Ask when you make the appointment. Dr. O is great!
Yes, proof of medical condition is required. You can bring the records with you if you have it. Otherwise, we can request it from your regular provider once you give us permission.
