Hi Sandrine, all you need to bring is your Florida Driver's License, any document stating your medical condition and/or any medication you take. If you do not have documentation or meds, we can simply request medical record directly from your diagnosing provider. Hope this helps
I saw Dr. O a few months ago & didn't need them but in my case, my qualifying condition was obvious. You might need them if your qualifying condition is chronic pain. Ask when you make the appointment. Dr. O is great!
Got a next day appt online. 150 to doc and 74 to state. Have your drs fax for supporting medical history.
