If the father is on the birth certificate they usually need both parents who are on that Birth certificate to sign off. Unfortunately you must have sole custody or be able to show valid pressing documentation on why you are unable to have the father agree/cooperate on signing off on your child's passport application. You can start with a petition for sole custody and gather up all the docs and evidence you have.
