If you’re an IL resident and you have a smart phone don’t forget that you can get your vaccination record set up to show in your smart wallet. The Illinois Department of Public Health’s website has the instructions to get it added.

Yes. Proof of Vaccination, ( photo of Vaccination Card accepted), & photo ID. Seating spacing, masks wasn't required at that time, but advanced notifications are timely.
