I passed my permit test and was given the paper copy back on September 17th, it is now almost mid December and I have still not received the hard copy of my permit card.

Ondrea, fill out the customer form you'll find here: http://www.dmv.ri.gov/feedback/ Include as much information as you can -- including your name, date of birth, address and the permit number. We'll have an administrator look at your record and advise you on the status of your license. It very well may have been returned to the DMV because of an incorrect address.
