We have a vehicle that has 1 license plate (the 2nd was damaged in an accident). How do we get a 2nd plate? Do we complete the Application for Registration (and if yes which sections do we complete) & Application for Remake of License Pates?

You must provide a police report for the lost/stolen plate. You can file the report at your local law enforcement agency. The smartest thing to do is apply for a remake when the plates come in when you go to the plate department bring your one good plate and the damaged plate or you can sign an affidavit that you lost the other plate
