Hello, you do not need to worry about obtaining a referral from your pain management doctor. Our staff will pull your information from E-FORCSE, which is Florida's prescription monitoring database and our physician will review that information in your consultation. Once your appointment is booked we email you over your intake forms, but you can also view them on our website CannabisDocGroup.com under the Patient Support tab.
