For a regular license, you will do a vision test and identify 9 out of 12 road signs. You only need to bring $40 cash, check, debit, or credit and your i.d. card. You'll take a new picture also. For real i.d.: you need a certified birth certificate or passport, social security card or a 1099 tax form, or a W2, as well as two documents with your name and address.
I'd recommend taking the documents you need to upgrade your license to Real-ID. I think you need your Birth Certificate, your Social Security card, and your Voter Registration card (if you have one). Verify your documents needs on the NC DMV website.
