You are required to bring a certified birth certificate, passport, or a school transcript for proof of birth; proof of social security such as a s.s. card or a w2 tax document; and proof of address with your name and address on it. The cost for i.d. cards are $13
ID with photo, 2 proof of address I think that all you need. But you can call to be sure.
I would suggest calling them to be sure.
