good afternoon, would a felony conviction of cruelty to animals in another state (killed a dog while he was training it) prohibit an individual from adopting a dog?

Yes! They ask that on the form. A person like that should NEVER be around animals. Dogs should never be trained with anything other than positive reinforcement.
