Hello, when I am filling out the passport renewal form, should I enter the Costa Rican or USA ID number and sign as it appears on the Costa Rican ID or the USA ID?

A Nicaraguan with a permanent resident visa in the United States is required to travel to Costa Rica.

.... there is only one ID and it is from Costa Rica, there is no US documentation that bears that name 😂
