I am with everyone else. Just go in or call and request them. But more then likely you will have to go in so they make sure they are giving the records to the correct person for HIPPA, reasons. It's for your protection.

Call the doctor office or go in there ask for the copy of your records is better if you go directly because you have to sign . Good luck
