The bus (air conditioned tour bus) takes you from the visitor center to the Apollo/ Saturn V center. The bus passes the NASA vehicle assembly building on the way. No full tour that I recall from several years ago. You are required to reserve a spot on the bus for your group as you enter the visitor center. Look for a board with a QR code as you walk from the parking lot to the entrance.
Yes, outside the gate they have a sandwich board with a QR code on it that takes you to the site where you can reserve a time to get on the bus. It wasn't super busy when we were there so we actually boarded about 30 minutes early.
Yes. We were there yesterday. You reserve a time for the bus it to the Saturn V and Apollo exhibits. Not sure about any tour other than that. You reserve the time online with your mobile device once you agree on property or at kiosk in the astronaut training experience building at the center. We had no problem getting a time.

(Do it while pulling in through the parking booths.) Try to get the earliest reservation. It starts 9:30am. You'll have the best photos opportunity. And you'll be finished there approximately 2hrs.
