You will have to sign in to virtual portal and resubmit your application. Due to the difference of address on your ID you will need to update your application online. After submitting your application it should take no more than 3weeks for it to be registered. Email will be sent acknowledging processing of application and then you can print temporary ID.....good luck 420👍🏽!
Your doctor's certificate expired in 2017. You need to make and appointment and get re-assessed.
