I go to Petcare, pretty sure most vets are pay upfront and put in a claim. Hope this helps.
Idk if this answer is going to help. Honestly, I don't it matters what kind of pet insurance you have because you still have to pay everything out of pocket and then send the bill to the insurance company and see how much they will reimburse depending on the kind of visit.
