We just went to Universal last week. My daughter-in-law did purchase the express passes AFTER we had purchased the regular tickets. She said it cost an additional $89 per ticket! She called Universal, I believe, and purchased them over the phone then made a quick stop as we went in to pick up the passes. Not cheap by any means. If you can get them BEFORE you get to Universal, you might save some money. Good luck!
Yes. I suggest purchasing it through the app. The app is great to tell you about rides what's running and what's not as well as your wait times

Yes. Just get there a little early and go to guest services just inside the entrance. The prices are different every day, check out the website for pricing
The universal express pass is $69.99 online (not sure about the tax) which is the cheapest price. You can go to the park and depending on the wait times get them there, but I expect you'd pay more... Maybe closer to the $89
