Please bring a list of medications (or bottles), your ID and insurance cards. You may also want to bring some sunglasses. Consider making a list of questions you have for the physician. When you make the appointment for the first time, they will inform you as to what to bring.
Depends on the Dr. But I usually bring all meds, referral, which should have been sent by a primary or referring Dr. Insurance card, the normal stuff.
