Buy the ticket at Costco, then take the receipt to a small office right behind the cash registers. They will give you the tickets there and there is a website to register the ticket(s). You can also register the tickets at universal, but it will take a little longer...
We had to register them online, each ticket to a specific person and register what days we went. I googled universal studio registration
