Start at the front desk.
On the providence website, there is a form you fill out to bring to the medical records office. Print and fill it out, bring it with you to the ER entrance. Stop at the security window on your way in, they may want to escort you to the basement. If that is not the case, continue to the registrar seated at the front desk and ask them for directions to medical records in the basement.
