Bay Cities will give you a form to fill out and for the Doctor's office to sign. You will receive $.25 per mile round trip on a Visa card. They require advance notice for reimbursement or transportation to your appointments.
You need to contact bay city brokerage and they will tell you how they can reemburse you for the mileage
