It's thru insurance and I use ohp it's only for med trans and doctor appointments
Bay Cities will give you a form to fill out and for the Doctor's office to sign. You will receive $.25 per mile round trip on a Visa card. They require advance notice for reimbursement or transportation to your appointments.
4 PM local time the day prior to the appointment in question.
