Size does matter it has to either fit under seat or in the overhead bin it will also count as one of your 2 faa mandated maximum for carry on and yes the icepack needs to be frozen solid
It doesn't matter the size. However if you are using an ice pack to keep it cold it needs to be frozen solid.
