As State Dept. said, you need your Social Security card (actual, not a copy) or your most recent tax year's W2 form (actual, not copy), your State ID, and a current piece of mail. All 3 must have the same address or you will be rejected from donation. Additionally there are weight restrictions & tattoo restrictions (call for more info). You mustn't be running a fever or feeling otherwise ill, and you can't be an IV drug user or engage in risky sexual behaviors (HIV/AIDs risk). You need to pass their physical exam. Your veins must also be able to handle a 17 gauge needle (which is rather large). They will not use smaller. So if you have small, shallow, "thread-like" veins you'll run a high risk of not being able to donate.
