Get tested and take the test certificate from DEQ to DMV to reregister.
Call DMV, ask them to email you the form. That's how I got mine.
I moved from out of state and didn't have the form. I paid a little extra at the DEQ for a VIN inspection. Then I had to make an appointment with the DMV to bring in my title & DEQ form in. You should just be able to go to the DMV with your DEQ form.
Contact your local DMV, magically work you're way through AI Support, then get to a live agent during business hours. Request a Re-issuing of your Registration Renewel Form. You technically don't need the form to have a DEQ test issued just the fee that they request. They will most likely give you a certificate in which you'll have to furnish to DMV at the time of registration. It's just much easier to have the renewal registration certificate with you at the time of DEQ. Keep in mind that the rates in the state of Oregon went up a little bit. For my newer vehicle the entire total cost was under $220 registration, tags and DEQ testing. I hope I helped in some way. Good luck everyone
